The coexistence of multiorgan ischaemia, recurrent thrombosis, and unusual vascular findings underscores the importance of maintaining a broad differential diagnosis and looking beyond conventional stroke mechanisms. This case illustrates how careful clinical reasoning and thorough etiological evaluation are essential for guiding both acute management and long-term secondary prevention.
