In stroke, diagnostic clarification of the underlying etiology is essential in order to determine the optimal secondary prevention strategy. This case highlights the significance of the (ischemic) infarct pattern, and its association with potential sources of cerebral embolism. The unexpected and informative results show how exciting the etiological classification of ischemic stroke and how instructive the individual treatment of each patient can be.
