Blood pressure (BP) management plays a critical role in patients with large vessel occlusion (LVO) undergoing mechanical thrombectomy (MT), as it directly influences cerebral perfusion, infarct progression, and the risk of hemorrhagic complications. During the peri-procedural period, inadequate BP control may compromise collateral flow before recanalization or contribute to reperfusion injury and intracranial hemorrhage after successful thrombectomy.

This webinar will review the pathophysiological rationale for BP control before, during, and after endovascular treatment, with particular emphasis on optimizing cerebral perfusion while minimizing the risk of hemorrhagic transformation and reperfusion injury.

First, Dr. Theodorou will present a real-world case example, that highlights challenges in clinical decision-making in this specific patient population. Next, Dr. Matusevicius will discuss current guideline recommendations, emerging evidence, and ongoing clinical trials, alongside practical considerations for individualized BP targets.
