Accompanying the articles, these visual summaries are crafted to convey the most important findings at a glance — helping readers quickly decide what to dive into.

Members of the ESO Young Stroke Physicians and Researchers Committee (YSPR) work with the authors and our medical illustrator to develop these visuals. We have received positive feedback on the visual abstracts published this year and would now like to identify the Top 3 through a community vote.

The YSPR has made a preselction of abstractsabstracts published in the ESJ over the past year. We invite all ESJ readers and ESO members to take part in the voting. Browse the 13 abstracts, rate each one from 0 (poor) to 5 (excellent), and help us pick the winner. Feel free to share the link with colleagues.

The winning visual abstract will be announced at ESOC 2026 in Maastricht.

We look forward to seeing you there!

Märit Jensen & Michele Romoli for the YSPR committee

It provides a safe and structured environment to train practical skills, improve decision-making, strengthen teamwork, and rehearse time-critical scenarios in acute stroke care. For junior clinicians as well as more experienced professionals seeking a refresher, simulation helps translate knowledge into action.

At ESOC 2026 in Maastricht (6–8 May 2026), the ESO Simulation Education Committee will host four Hands-on Simulation Workshops on Thursday, 7 May 2026. We will simulate two acute stroke cases in a simulated emergency department environment with you actively role-playing the physician part. These small-group sessions are designed to provide practical, case-based learning with direct faculty feedback, and places are limited. Workshops can be booked separately during ESOC registration on a first-come, first-served basis.

Here is what to expect from each workshop:

Workshop A | 08:30–10:00 – Simulation Workshop on Code Stroke: From Diagnosis to Early Treatment

A practical workshop on acute stroke diagnosis, early treatment, door-to-treatment times, and crew resource management in the hyperacute setting. Particularly suited to participants looking for a structured overview of the code stroke pathway, either as a starting point for simulation-based training or as a focused refresher of core principles.

Workshop B | 13:15–14:45 – Simulation Workshop on the Management of the Deteriorating Patient After Reperfusion Therapy

Focused on the acutely deteriorating patient after reperfusion therapy, with emphasis on decision-making, communication, shared mental models, and interprofessional learning. Especially relevant for participants interested in teamwork and crisis management in a high-pressure scenario.

Workshop C | 15:00–16:30 – Simulation Workshop on the Management of Complex Acute Stroke Cases

Dedicated to challenging stroke presentations, including complications, frailty, and contraindications to treatment. Particularly suitable for participants with prior experience in acute stroke care who want to deepen their approach to complex cases and nuanced treatment decisions.

Workshop D | 17:00–18:30 – Simulation Workshop on Tele-Stroke Management in the Acute Stroke Setting (Joint Session of the ESO Stroke Simulation Committee and the ESO Telestroke Committee)

This workshop explores simulation training in a telestroke environment, including remote assessment, communication, NIHSS scoring, workflow optimisation, and complication management using telemedicine equipment in a controlled simulated setting. Especially relevant for participants interested in understanding the operational principles of tele-stroke services and gaining initial experience within such an environment.

As each workshop takes place in a different time slot, participants are welcome to register for more than one session and build a progressive learning experience across the day.

Whether you are early in your stroke career or looking to refresh essential practical skills, these workshops offer a unique opportunity to learn by doing.

Places are limited! Book your workshop during ESOC registration and join us in Maastricht, Thursday 7 May 2026: #VoiceOfStroke.

The vision was ambitious—combining advanced techniques including omics with machine learning to improve prognostic accuracy in a such a rare and complex cerebrovascular disorder. Our team invested months in designing the study, building collaborations, and drafting a comprehensive grant proposal. I was confident this project was successfully funded allowing to improve personalised medicine in Moyamoya.

The outcome, however, was not what I expected. The funding application was rejected. The initial reaction was a mix of disappointment and self-doubt. When a project you deeply believe in is turned down, it feels like a personal failure. But after the first wave of frustration, I realised that resilience in research is not about avoiding setbacks—it’s about transforming them into stepping stones. I carefully reviewed the reviewers’ comments and understood that the main concern was the proposed team strenght and feasibility. In fact the proposal lacked preliminary data to support the integration of AI models with biomarker analysis.

Instead of abandoning the idea, we shifted strategy. We started creating an Italian network of centers involved in clinical and reaserch of Moyamoya disease to collect a dataset of clinical and biological data and evaluated molecular markers in a smaller cohort. This process not only strengthened the scientific foundation but also expanded our network of collaborators, including data scientists and bioinformaticians. Months later, we resubmitted a revised proposal—and this time, it was funded. Today, the project is generating insights on molecular biomarkers of Moyamoya disease and we are working on the prediction model development that may help predict outcomes and guide treatment decisions in Moyamoya disease.

What did I learn? First, rejection is not the end—it is feedback. Second, resilience is not passive endurance; it is active adaptation. In research, flexibility and persistence are as important as innovation. Finally, collaboration is key: complex challenges require multidisciplinary approaches and open minds.

As Chair of the ESO Education Committee, I often remind young researchers that science is rarely a straight path. It is a journey with detours, and those detours teach us how to navigate better. Embrace setbacks, learn from them, and keep moving forward—because resilience is the true engine of discovery.

A major highlight of 2025 was ESOC in Helsinki, Finland. Once again, the conference provided an excellent platform for scientific exchange and networking among stroke professionals from across Europe and beyond. The YSPR Research Design Workshop was well received, giving young researchers the opportunity to present their study ideas and receive structured, constructive feedback from experienced mentors.

Together with the European Stroke Journal (ESJ), the YSPR Young Reviewer Programme continued successfully in 2025. The participants for the current round have been selected, and this cycle will conclude in April 2026. Building on the strong interest and engagement seen so far, the call for new candidates for the ESJ Young Reviewer Programme will open after ESOC 2026.

The Department to Department Programme continued to offer valuable opportunities for young stroke physicians to experience different clinical and research environments across Europe. Those interested in applying are encouraged to read the previous participant reports published on the ESO blog, which provide insightful first-hand perspectives on the programme.

The ESO blog continues to be an important platform for presenting current topics and sharing perspectives. Working on the blog remains a great part of our committee work. We would like to thank everyone who contributed over the past year, and we are always happy to welcome new guest bloggers with fresh views on timely and relevant stroke-related topics.

Looking ahead to 2026

The coming year will mark the start of a new YSPR initiative: Back to Bedside. This programme aims to present short, focused clinical vignettes with clear take-home messages, highlighting the importance of clinical reasoning and bedside neurology in everyday stroke care. In case you have missed the first episodes, you can listen here to Episode 1 and Episode 2

Finally, we would like to thank all ESO members, collaborators, mentors, and participants for their continued support and engagement.

We wish everyone a successful 2026 and look forward to seeing you in person in Maastricht at the 12th ESOC!

The ESO Guideline Board invites experienced stroke clinicians and subject-matter experts to join the Delphi voting panel for the ESO White Paper on PRES RCVS in development. We are seeking Delphi panellists who can contribute their expertise by evaluating and voting on draft consensus statements relating to the defined PICO questions of the project.

Role of Delphi Panel Members

Delphi panellists will:

Panellists will not be responsible for authoring text or conducting literature reviews – their role is strictly to evaluate the proposed guidance through structured voting.

Introduction to the ESO White Paper Format

The European Stroke Organisation (ESO) White Papers serve as expert consensus–based clinical guidance in areas of stroke care where robust evidence is limited, evolving, or currently insufficient to support full evidence-based guideline recommendations. As outlined in the ESO Standard Operating Procedure (SOP), White Papers use a transparent, structured, and iterative consensus method, drawing on multidisciplinary expertise to formulate practical clinical guidance for high-priority topics.

A central component of developing White Papers is the modified Delphi process, an established method designed to gather expert judgement through multiple rounds of anonymous voting, structured feedback, and iterative refinement. This ensures that all expert perspectives, irrespective of seniority, geography, or discipline, are weighted equally and transparently in formulating consensus statements.

To uphold the methodological rigor described by ESO, the Delphi panel must include a sufficiently broad and diverse group of experienced clinicians from across Europe and beyond.

Eligibility Criteria

To ensure a high-quality and diverse panel (minimum 20 active voters), applicants should:

Have at least 10 years of clinical experience (10 years since graduation from Medical School), preferably in stroke medicine or a related neurological or interdisciplinary field. The same requirement of 10 years of experience after graduation also applies to voting members of the ESO Guidelines MWGs (Module Working Groups).

Possess recognised expertise relevant to the White Paper’s clinical topic.

We particularly welcome applications from a broad range of clinical backgrounds, geographic regions, and career stages to ensure a balanced representation. Participation is voluntary, not reimbursed and does not confer authorship.

Why Participate?

Serving as a Delphi panellist contributes directly to shaping high-quality, internationally visible clinical guidance. Your input will:

The result? A warm conference icebreaker and a vibrant networking hub for stroke researchers in the mid-career stage.

The afternoon opened with ESSW Chair Rustam Al-Shahi Salman, followed by co-chairs Signild Åsberg and Annemijn Algra, who led two mid-career groups into the first session: an elevator-pitch challenge. Volunteers presented a strictly timed three-minute pitch (three slides maximum) about their upcoming research. The organising committee scored each pitch on originality, potential impact, and clarity, while peers provided one “tip” and one “top” on paper notes. The concise format sparked lively debate, enthusiastic feedback, and several promising ideas for collaboration—proof that overlapping research themes quickly build bridges.

After a short break, the group reconvened for an introduction to narrative CV writing. Signild and Annemijn set the tone by asking participants to share a recent achievement they were proud of and the impact it created. The room filled with conversations—some of those achievements were not visible in conventional metric-based CVs. Using provocative statements and historical examples (including 18th-century precursors), the group discussed strengths and limitations of narrative CVs. A key takeaway: hybrid formats—such as the Luxembourg National Research Fund template—may be a pragmatic way forward, blending metrics and narrative in a concise and transparent manner.

The closing session was an intimate conversation with Gabriel Rinkel, world authority on subarachnoid haemorrhage and intracranial aneurysms. Rather than standing at the front of the room, he sat among the participants and shared honest lessons from his career as a clinician, teacher, researcher, and head of department. From strategies to find focused writing time to receiving—and later providing—mentorship, Gabriel emphasised one recurring message: international collaboration and friendships shape not only our scientific impact, but also our happiness in academic life.

Before closing, Gabriel announced the winners of the elevator-pitch challenge:

Jatinder Minhas (United Kingdom) — “Protective and Reparative Processes After Acute ICH – Can We Recruit These or Do They Present a Risk?”

On behalf of the ESSW organising committee, we extend our thanks to all mid-career researchers for their preparation and active participation. You turned this inaugural workshop into a genuine success—one that will undoubtedly return at ESSW 2027.

It was a unique blend of high-level science, practical training, and cultural exchange — all set within the historic vibe of one of Europe’s most ancient cities. From the very first day, the atmosphere has been welcoming and inspiring. We were reminded right away that stroke medicine is truly multidisciplinary. The early sessions on stroke unit care set the stage, emphasizing how essential ESO stroke unit certification is to establishing a highly professional, recognised, and valuable center for stroke care. Meanwhile, the “Heart and Brain” lectures revealed the complex interplay between cardiology and neurology — from atrial fibrillation and heart failure to PFO-associated strokes and left atrial appendage occlusion. These lectures highlighted how deeply interconnected our fields are and how advances in one specialty directly influence patient outcomes in another.

One of the things that made this sense of connection even stronger was our WhatsApp group, an open space where everyone could ask questions, share information, and stay in touch. It quickly became an essential part of the experience, and the best part is that it will stay active even after the Summer School, keeping the whole group connected. The interactive case discussions quickly became a highlight of the course. Using live voting,
we were challenged to think through real-life scenarios, compare perspectives, and then learn from expert feedback. It created an energy in the room that kept us engaged long after the sessions ended.

Day 3 of the Summer School was especially memorable. On that day, we had the privilege of learning directly from members of the ESO Executive Committee, world-leading physicians and researchers who are shaping the future of stroke medicine at the highest level. To hear them present updates on thrombolysis, stroke care systems including mobile stroke units, new technologies used in rehabilitation, artificial intelligence – it’s current and future role in stroke management as well as cerebral venous thrombosis management was both humbling and motivating. These lectures did not just give us knowledge — they gave us a sense of being connected to the cutting edge of the field, and of belonging to a community that is pushing boundaries worldwide. It felt like a true highlight of the week, and one we will not forget. The hands-on workshops were another unforgettable moment. At Attikon University Hospital, rotating between neurosonology practice and endovascular simulation was a chance to put theory into practice, guided by experts. Later in the week, at the Attica Rehabilitation Center, we explored botulinum toxin injections, transcranial magnetic stimulation, and exoskeleton-assisted therapy — a reminder that stroke care does not end with reperfusion, but continues into rehabilitation and long-term recovery. Beyond lectures and workshops, the social programme organised by the local team helped us create memories and connections with other peers from the same field that were just as valuable. Walking together through the narrow streets of Plaka on the first evening immediately broke the ice, and dinners at traditional tavernas or by the sea became opportunities to bond and form real friendships. The guided tour of the Acropolis Museum was breathtaking, and the farewell dinner at Mikrolimano Port, overlooking the water, felt like the perfect way to close the week. These moments reminded us that medicine is also about human connection and that we are all working towards the same goal – to ensure better care for our stroke patients.

Looking back, what stays with us is not just the knowledge we gained but the sense of belonging to a wider community. Stroke care is one of the best examples of multidisciplinary medicine, and throughout the week we saw how neurologists, radiologists, cardiologists, neurosurgeons, and rehabilitation specialists can come together, each bringing their unique perspective to improve patient care.

We are deeply grateful to the European Stroke Organisation, to Professor Tsivgoulis, Lina Palaiodimou and the local organising committee, and to the incredible faculty for their effort, warmth, and dedication. The 28th ESO Summer School was more than just an academic course — it was a journey of learning, inspiration, and friendship. But still, after this whole high-level scientific week, one question remains unanswered and still lingers — what really is the perfect temperature for a lecture room? Too cold? Too warm? Those who were there will understand… and for everyone else, feel free to ask, we’ve got stories to tell! And for those who want stories of their own, apply for the next ESO Summer School!

This is a focused workshop for early career investigators.

The aim of this workshop is to support young stroke researchers in promoting their proposed or ongoing research project to the ESOC audience. They will be given the opportunity to present their work and receive feedback from an expert mentor. The goal of this feedback is to allow young researchers to optimise their study design.

As in previous years, young investigators (≤35 years) will be invited to submit an abstract for the workshop describing their research project, preferably a project still under development or interim results from an ongoing project. Abstracts can be submitted to ESOC as a special category ( “4.28 YSPR – Study Design Workshop”).

The successful applicants will give an oral presentation, followed by constructive assessment of the study’s design and advice on its future development by a senior, internationally renowned invited investigator. In addition to providing expert guidance on the development of the study, the workshop will be an opportunity for the young researchers to promote their work to the wider community and establish collaborations.

The presenters of the two top scoring abstracts will receive a place at the ESO Summer School or the ESO Edinburgh Stroke Research Workshop . All presenters will be interviewed by other Young Stroke Physicians and Researchers and the interviews will be featured on the ESO blog.
