<div  class='flex_column av-23890mm-97def1f5d0ed263cc1cb53827f6ef0da av_one_full  avia-builder-el-3  el_after_av_textblock  el_before_av_one_full  first flex_column_div  column-top-margin'     ><section  class='av_textblock_section av-ob3d0u-33c737abfe8a451756aba2b3442a20bf '   itemscope="itemscope" itemtype="https://schema.org/BlogPosting" itemprop="blogPost" ><div class='avia_textblock'  itemprop="text" ><p>The first ESO Outreach Regional Meeting took place on June 6<sup>th</sup> 2024 in Sinaia, being organized as a full morning session, during the 22<sup>nd</sup> National Congress of the Romanian Society of Neurology.</p>
<p>The initiative belonged to Peter Kelly during his ESO presidency, and the further details regarding the program were strongly supported by Simona Sacco and Guillaume Turc, being decided together with Cristina Tiu, president of the Romanian Society of Neurology. The themes were selected with the purpose to cover the most important and recent achievements regarding stroke treatment (both ischemic and hemorrhagic) and to deliver the knowledge towards people who did not have the opportunity to attend ESOC in Basel.</p>
<p>The program included recent results from trials involving Intracerebral hemorrhage, clearly and eloquently presented by Thorsten Steiner. The program then followed the stroke patient`s pathway, from Prehospital models of organizing the activity (Silke Walter), to Updates on intravenous thrombolysis (Guillaume Turc), updates on mechanical thrombectomy (Marios Psychogyos) and on secondary prevention (Lina Palaiodimou). Arne Lindgren covered the most important aspects regarding small vessel disease, and Michal Karlinski and Francesca Romana Pezzella made a comprehensive presentation of ESO-EAST project and respectively of SAP- E, stressing out the achievements of Romania regarding the rate of IVT or the number of stroke ready hospitals, but also the importance of reaching the established KPIs until 2030.</p>
<p>The session ended with a short ceremony for the acknowledgement of the Romanian Hospitals which received Gold (12), Platinum (2) and Diamond (2) status awardees from ESO- Angels` Initiative. In 2017 there was only one hospital in Romania which achieved the gold status. Today more and more hospitals are improving, and we also have one application for ESO Certification, which we hope it will be successful.</p>

<p>The number of participants was very large, 800 participants registered on site, and almost 450 registered online.</p>
<p>The feedback of the event was very positive, with great appreciation for the lectures and also for the speakers, each lecture was followed by questions from the audience, and the networking continued during the breaks.</p>

<p><strong>ESO EAST Workshop – the 10th Anniversary of ESO EAST</strong></p>
<p>The ESO EAST Steering Committee organized the ESO EAST Workshop, supported by the ESO EAST Head Office.</p>
<p>The 10th Anniversary ESO-EAST workshop was organized on 14th May 2024., traditionally as a pre-conference meeting, a day before ESOC  2024 in Basel, Switzerland. It gathered representatives from ESO EAST countries, the ESO EAST Steering Committee, ANGELS Initiative representatives, the RES-Q Team, the ESO Head Office, industry partners, and guests. This year, we were honored by participating in three ESO presidents: Peter Kelly, ESO president; Simona Sacco, ESO President-elect; and Valeria Caso, former ESO president and an ESO EAST founder. They gave an opening speech regarding the ESO mission and the role and future of ESO EAST. After opening speeches, founding members of ESO EAST – Rober Mikulik, Natan Bornstein, Axel Kolmetz, Veronika Svobodova, and Valeria Caso presented the history of ESO EAST and presented their remembrance of ESO EAST&#8217;s founding.</p>
<p>The workshop was divided into two parts, one chaired by the new ESO EAST Steering Committee members (Zuzana Gdovina, Adam Kobayashi, Cristina Tiu, and Hrvoje Budincevic) and the other introduced by Francesca R. Pezzella.</p>
<p>The workshop had two special lectures: Pawel Swieboda gave a speech on European Brain Health Policy and European Health Data Space (as a Key lecture). Milan Vosko presented change management as a tool for improving door-to-needle time.</p>
<p>The goals of this year&#8217;s workshop were to:</p>
<ol>
<li>Share novel features on the RES-Q registry.</li>
<li>Present practices on monitoring of stroke care in selected ESO EAST countries.</li>
<li>Present ESO EAST projects and publication plans.</li>
<li>Improve collaboration with ESO Committees.</li>
</ol>
<p>Rupal Sedani, Cristina Tiu and Robert Mikulik presented novel features and the future of the RES-Q registry. Colleagues from Estonia, Hungary, Georgia and Moldova presented their practice for stroke care monitoring. The more collaboration between the ESO EAST and ESO Committees is expected, colleagues from selected ESO COmmittes presented their plans – Tatjana Kharitonova (for AI Committee), Zuzana Gdovinova (Brain and Heart Committee), Anita Arsovska (Education Committee), Zdravka Poljakovic (PR Committee), Aleksandras Viloskonis (Stroke Unit Certification Committee) and Janika Korv (Neuorehabilitation Committee). Jan van der Merwe presented collaboration with Angels Initiative. Particular focus was put on joint research papers on dysphagia (Michal Karlinski), access to rehabilitation (Anita Arsovska), reperfusion therapy (Zuzana Gdovinova), Stroke units (Hrvoje Budincevic), and Baltic countries stroke quality paper (by Aleksandras Vilionskis).</p>
<p>Another particular topic was presentations on projects from and of ESO EAST countries:  application for management of EVT in Poland (Adam Kobayashi). Some of the topics needed more time for discussion, and they will be covered by webinar, e.g. mobile stroke units in Czechia (Robert Mikulik), harmonizing hospital reports of stroke patients (Milan Vosko) and rehabilitation training program (Francesca R. Pezzella and Axel Kolmetz). Discussions were conducted in an amicable and constructive atmosphere, as it is a practice in all this ten years.</p>

<p><strong>ESO EAST Workshop</strong></p>
<p>The ESO EAST Workshop was organized by ESO EAST Steering Committee, supported by ESO EAST Head Office.</p>
<p>The workshop is traditionally organized a day before ESOC, as a face-to-face meeting/workshop. It gathered together representatives from ESO EAST countries and the ESO EAST Steering Committee, ANGELS Initiative representatives, RES-Q Team, ESO Head Office, industry partners, and invited guests. The goals of the meeting were to: share updates on ESO EAST countries and partners, presentation of the RES-Q results, and publication plan, define possible improvements of RES-Q, and prepare the first draft of the harmonized discharge summary report.</p>
<p>The 2023 Workshop was chaired by Valeria Caso, Francesca R. Pezzella, Natan Bornstein, Veronika Svobodova and Robert Mikulik. Peter Kelly, ESO President, opened the workshop with an overview and plans for ESO. Particular focus was put on the Registry ofStroke Care Quality (RES-Q) &#8211; www.qualityregistry.eu which became a global stroke registry for monitoring the quality of stroke care that was presented by Robert Mikulik.</p>
<p>Representatives from 21 countries have met to share their achievements, progress, and raising issues by exchanging experiences in stroke care improvements in their countries. Current projects (IRENE, RES-Q+, ANGELS) and future projects (stroke rehabilitation) were presented. The current collaboration between the ESO EAST team with Angels Initiative representatives was reported and briefly discussed. Particular focus was put on discussing possibilities of RES-Q interactive dashboard improvement, using artificial intelligence for RES-Q data feedback (led by Hendrik Knoche). Another special topic was harmonizing hospital reports of stroke patients with drafting the first proposal for a unified discharge report (led by Milan Vosko). Discussions were conducted in a very friendly and constructive atmosphere.</p>

<p>After several online meetings regarding Stroke Action Plan for Europe (SAP-E) on May 3rd, 2022 the SAP-E Workshop held as a face-to-face meeting of leadership and representatives involved in this project. Francesca R. Pezzella, Robert Mikulik, and Valeria Caso were moderators of the Workshop. Bo Norrving presented the overview of SAP-E, and Hanne Christensen reported the implementation of SAP-E. Arlene Wilkie, representing SAFE,  presented the lay perspective of SAP-E.</p>
<p>Guidelines and essentials of stroke care were presented by Thorsten Steiner. Cristina Tiu shared their experience in Romania in improving stroke care and the opportunities of SAP-E. Robert Mikulik shared experiences from ESO EAST that would be useful for SAP-E implementation.</p>
<p>SAP-E success stories were presented by Yuriy Flomin (Ukraine) via video, Ana Catarina Fonseca (Portugal), Fabrizio Pennacchi (Italy), and Nune Yeghiazaryan (Armenia).</p>
<p>The Presidents of ESO (Michael Dichgans)  and SAFE (Hariklia Proios) spoke on the importance of SAP-E for both of their organisations, stakeholders and stroke care in Europe. They presented awards to representatives from 6 countries (Bulgaria, Estonia, Italy, Germany, Greece,  and Serbia)  based on significant and exemplary efforts to support, lead, report, and promote the SAP-E in their countries.</p>

Nearly 40 representatives from ESO EAST countries came together with the ESO EAST Steering Committee, members of the SAP-E Implementation Committee, and industry partners. This workshop is traditionally organised a day before ESOC, as a face-to-face meeting/workshop. As in previous years representatives from 21 countries have met to share their achievements, progress, and raising issues by exchanging experience in stroke care improvements in their countries. ESO EAST is the comprehensive international program for improving stroke care in Europe, initiated by the ESO in 2015 initiated by former ESO President, Valeria Caso and ESO EAST Chair, Robert Mikulik. The 2022 Workshop was chaired by Valeria Caso, Francesca R. Pezzella, Natan Bornstein and Robert Mikulik. Peter Kelly, ESO President-elect visited the workshop and stressed the role of exchange of experience and the value of ESO individual membership. Special focus was put on the Registry of Stroke Care Quality (RES-Q) which became a global stroke registry for monitoring the quality of stroke care. Achievements in ESO EAST countries were discussed in a very friendly and constructive atmosphere. Current projects (IRENE, RES-Q+, and V4H) and future projects (stroke rehabilitation monitoring) were presented, The current collaboration between the ESO EAST team with Stroke Action Plan for Europe (SAP-E) representatives and Angels Initiative representatives was reported and a brief discussion about future possibilities followed. Experience from improving stroke care in Ukraine and Azerbaijan and the task force for Ukraine was presented separately. Fociused groups discussed elements of the RES-Q registry. The RES-Q Registry is open to all countries, which are interested to improve their stroke care. All are welcome to join the project on www.qualityregistry.eu. <p>The ESO EAST Program started in 2015 as an initiative to improve stroke care quality in the following Eastern European countries: Albania, Armenia, Azerbaijan, Bosnia, Bulgaria, Croatia, Czech Republic, Estonia, Georgia, Hungary, Kazakhstan, Kyrgyzstan, Lithuania, Macedonia, Moldova, Poland, Romania, Russia, Serbia, Slovakia, Slovenia, Ukraine and Uzbekistan.</p>
<p>After selecting the ESO EAST international Steering Committee and National Representatives, several further steps were outlined: 1) establishing a leadership, i.e. setting a national steering committee to implement a stroke care improvement program in their respective countries, 2) defining stroke hospitals, 3) monitoring stroke care quality in these hospitals through quality and performance metrics collection in a registry, and 4) developing a national stroke care improvement strategy.</p>
<p>National Steering Committees were established in 19 ESO EAST countries, a registry for quality monitoring was developed, and quality monitoring was initiated in many hospitals throughout the participating countries.</p>
<h3></h3>
<h3><strong>Registry in stroke care quality: RES-Q</strong></h3>
<p>RES-Q was developed as tool for quality monitoring. During development, emphasis was placed on ease of data collection and availability of online feedback. Started in Eastern Europe, after 3 years RES-Q is used for monitoring stroke care quality by 1.300 hospitals in 70 countries around the world. RES-Q contains data on nearly 250.000 stroke patients with nearly half from ESO EAST countries. Since 2018, more than 25 posters showing the results of stroke care quality monitoring have been presented by ESO EAST countries at ESOCs.</p>

<p>The ESO EAST Steering Committee constantly puts an effort into obtaining funds from European grants. In 2019 we succeeded in winning a grant from the COST Association (Cooperation in Science and Technology) to support research in stroke care quality in ESO EAST countries and beyond. Several ESO EAST delegates were elected leaders of this project, such as D. Bereczki (Hungary), C. Tiu (Romania), S. Andonova (Bulgaria), A. Arsovska (North Macedonia), M. Karlinski (Poland), and S. Groppa (Moldova) and have been able to plan complimentary training programs.</p>
<h3><strong>Meetings in ESO EAST countries bring opportunities</strong></h3>
<p>One of activities supported by the COST grant was a meeting in Moldova in September 2019, where 60 representatives discussed steps to build an evidence base in stroke care quality in Eastern Europe and identified several research projects to be conducted. This meeting was crowned by an audience with Moldavian president, Igor Dodon. ESO EAST Representatives emphasized the importance and value of focusing on improvements in stroke care in the country, and the region.</p>
<p>Unfortunately, many activities funded by the COST grant, such as training schools, meetings, and study stays for ESO EAST participants had to be cancelled for 2020/2021 due to the Coronavirus pandemic.</p>

<h3><strong>Simulation training became popular in ESO EAST countries</strong></h3>
<p>In 2016 we developed a simulation training program in acute stroke treatment to help stroke physicians and nurses in ESO EAST improve their knowledge and skills in providing thrombolytic treatment. In Brno, Czech Republic, we trained approximately 300 physicians and nurses from 10 ESO EAST countries. We have also supported stroke champions in Russia and Ukraine in developing their own national simulation programs in acute stroke treatment. This led to establishing stroke simulation centres in St. Petersburg (Russia) and in Poltava (Ukraine) where stroke teams from their regions are trained.</p>
<p>In January 2020, we published an ESO EAST Program methodology and its results in the European Stroke Journal. We believe the stroke improvement program is scalable beyond ESO EAST countries, and is valuable to many other regions facing similar challenges.</p>
<h3></h3>
<h3><strong>Synergy with European Stroke Action Plan is our priority</strong></h3>
<p>In the coming months, we are going to arrange six virtual workshops where ESO EAST delegates will be developing implementation strategies of the Stroke Action Plan for Europe 2018–2030 tailored to Eastern countries. The creation of the strategies will be facilitated by ESO EAST Steering Committee members, who are involved in the Committee for Implementing the Action Plan as well. These plans will be introduced in an all-ESO-EAST-members virtual meeting at the end of 2020 and should be implemented from the beginning of 2021.</p>
<h3><strong>Thanks for supporting ESO EAST</strong></h3>
<p>We strongly appreciate our ongoing cooperation with the ANGELS Initiative, as our synergy makes stroke care improvement in ESO EAST countries faster and more feasible.</p>

<p>In 2015 European Stroke Organisation (ESO) launched the ESO EAST (European Stroke Organisation – Enhancing and Accelerating Stroke Treatment) Project. The aim of this project was to facilitate the implementation of new proven approaches in stroke diagnostics, management and treatment. ESO EAST Project includes not only countries from Eastern Europe (Estonia, Lithuania, Latvia, Slovenia, Czech Republic, etc.), but also countries from CIS and Central Asia: Georgia, Armenia, Kyrgyzstan, Kazakhstan and Uzbekistan. It is well known that the approaches for stroke management are different in Eastern European and Central Asian countries compared to the rest of Europe, not only in terms of  the availability of internal resources for the establishments of stroke unit care but also the implementation of established acute stroke treatment. In some member countries of the ESO EAST Project, the thrombolytic therapy in ischemic stroke is not yet implemented, or if started, the pathway still requires reorganization so that the service can improve from single cases to more of a routine.</p>
<p>ESO EAST is the first project which united all mentioned countries in one aim: to provide stroke patients an equal access to evidence-based stroke care.</p>
<p>Before implementing and bringing changes in a country’s healthcare it’s always crucially important to analyze the current situation and needs of the stroke physicians, hospitals and governments. Establishing the united stroke quality monitoring was an important milestone for the ESO EAST Project. The monitoring of stroke care quality systems became possible with the implementation in ESO EAST member countries of the Registry of Stroke Care Quality (RES-Q) which has free access, is user-friendly and not time consuming. In parallel, RES-Q includes important parameters such as “door-to-needle time” and “door-to-groin time”, and use of antiplatelets, oral anticoagulants as preventive measures, during the hospital stay and after discharge, which can reflect the country’s situation in stroke management and show the fields that requires improvements.</p>
<p>Consultants from an independent organization, Angels Initiative, are providing educational and funding support to member countries to practically facilitate implementation of changes. With the help of Angels Initiative, thousands of nurses, physicians, students, residents and healthcare specialists already received the access to online modules, BODYINTERACT cases with an electronic stroke patient treatment, The Angels WOW CT Training and many others.</p>
<p>Kyrgyzstan joined ESO EAST Project in 2015, RES-Q – in 2016 and Angels Initiative Project – in 2017. Dr.Inna Lutsenko has been coordinating the Kyrgyz branch of ESO EAST since 2015. Since then 15 doctors and volunteers have been trained on “Train the trainer” led by Angels Initiative in Germany. Dr.Lutsenko’s team trained 12 stroke departments (90 physicians) and 10 stroke departments were included in the RES-Q registry. Kyrgyzstan team presented the RES-Q findings on the annual ESO EAST workshops and during ESO conferences. We implemented for population an official first stroke recognition algorhythm “<a href="https://www.morressier.com/article/first-fast--sokku-stroke-recognition-campaign-kyrgyzstan-successful-implementation-fastlike-adapted-algorithm-sokku-raising-stroke-awareness-population/5cb58cfdc668520010b56d56" target="_blank" rel="noopener">СОККУ</a>”, Kyrgyz analogue of FAST, supported by Ministry of Health and since November 2019 we introduced the <a href="http://www.med.kg/en/news/494-evidence-based-algorithms-and-scales-for-acute-stroke-diagnosis-have-been-introduced-by-the-eso-east-project-volunteers-at-the-emergency-medicine-center-in-bishkek.html" target="_blank" rel="noopener">Evidence-based Algorithms and Scales for Acute Stroke Diagnosis at the Emergency Medicine Center in Bishkek</a>.</p>
<p>Kyrgyzstan is an example how, with the thorough guidance of the ESO EAST Steering Committee, one can take the first steps to implement the best and evidence practices by starting from the education of the general population, emergency centers personnel and working with the efficient RES-Q registry.</p>

In October 2018 a hospital in Romania started working with Angels. Before then they were not able to treat stroke patients according to guidelines. In the first month since starting with Angels, a fellow medical professional suffered a stroke. If he had his stroke just one month earlier, his outcome may have been very different.<br />

<p>IRENE COST Action Meeting Report to ESO</p>

<p>The Management Committee Meeting of IRENE COST Action took place in Chisinau, Moldova, 16th-17th of September 2019, hosted by Prof. Stanislav Groppa, attended by 46 delegates from 26 countries (many FESO and ESO-EAST representatives), chaired and co-chaired by Prof. Robert Mikulik and Prof. Natan Bornstein, respectively. Based on the principles of ESO-EAST platform which was created by Prof. Valeria Caso and the efforts of the Steering Committee, and also formation of RES-Q database, IRENE stands for Implementation Research Network in Stroke Care Quality. It is supposed to support, accelerate and enhance the measurement of quality of stroke care, in countries where it has never been done before.  IRENE COST Action is designed to provide a solid base to collect and share data for countries, which are at varying stages of implementing modern stroke treatment programmes. As Veronika Svobodova (Grant Manager) stressed, the main objective is to implement existing treatments in acute stroke more effectively through the development of an implementation framework and through the testing of the implementation effectiveness. The IRENE COST Action has a high geographical relevance main focus on countries, where the burden of stroke is higher, while quality of stroke care is lower (mainly Eastern Europe and Central Asia). IRENE COST Action will pioneer a large-scale stroke care quality improvement project, which will: improve understanding of contextual factors, lead to improved uptake of proven interventions, decrease disparities in stroke care (mainly in Eastern Europe).</p>
<p>Main points of this meeting were to introduce the research proposals and to choose with voting which proposals are fundable from IRENE after scientific discussion. Delegates also had the opportunity to show interest and join one of the working groups that were previously defined during the first meeting in Brussels in April this year. The four Working Groups are the following: WG 1 – Leadership (provides scientific leadership, responsible for development and management of the network, consociation of members, acceleration of knowledge exchange in leadership and establishment of leadership in stroke care in focus countries);  WG 2 – IRENE COST Action Members (responsible for development of a national network for stroke care quality measurement, involvement of national stakeholders, creation of a national implementation strategy); WG 3 – Registry Management (from research concept to implementation: description of the methodology and technical solutions, platform enhancement focused on meeting diverse country demands, automated inclusion of data points from existing registries, appropriate and up-to-date security, alignment with applicable legal frameworks) and WG 4 – Dissemination (responsible for enhancing the impact of the IRENE COST Action through: a) the involvement of diverse stakeholders b) information activities targeted at stroke professionals and stakeholders in other geographical areas c) campaigns to motivate professionals in other medical fields to follow the IRENE COST Action results; informing the lay public about stroke risks and stroke care, in order to increase their awareness and potential interest to be involved in the implementation process.</p>
<p>Overall, the two days intensive meeting was an excellent opportunity to meet with colleagues, make plans for future collaboration, exchange knowledge, and also and great inspiration to continue to work in order to improve the quality of stroke care in each country involved. All delegates enjoyed the warm hospitality of Prof. Stanislav Groppa and his team and were delighted by the charming city of Chisinau and its natural beauties.</p>
<p>As Dr Francesca Romana Pezzella (Science Communication Manager of IRENECOST Action) said: &#8220;IRENE network is a think tank! COST ACTION 18118 a valuable tool for empowering the quality of stroke care in the European countries, offering the opportunity to be in contact with an extremely lively, proactive and experienced scientific community. IRENE supports meetings, workshops, training schools which are critical to establish  the culture of quality of care&#8221;.</p>
