<p>While the pandemic continued to disrupt our world in 2021 and health care workers dealt with another year of combatting the coronavirus, there was also reason for hope. In celebration of our strongest weapon against COVID-19, TIME magazine choose the scientists that lead the development of the COVID-19 vaccines as Heroes of the Year 2021. Also, for the stroke community, there was positive news from all over the world on how health education and improvements in management could reduce the persisting impact of the pandemic in the delivery of care to stroke patients.</p>
<p>The many interesting and inspiring scientific experiences developed by ESO during this year were also a demonstration that we learnt how to ensure effective scientific exchange and education, even though the pandemic continued to force us meeting online.</p>
<p>At the beginning of this year, the 8th ESO ESMINT ESNR Stroke Winter School was held. A total of 96 selected participants from all over the world gathered to listen to talks from leading neurologists, interventional and non-interventional neuroradiologists, neurosurgeons, and neuropaediatricians. They shared their knowledge and views on interdisciplinary management of stroke patients.</p>
<p>Despite the fact that our scientific conference was again entirely online, ESOC 2021 was an extremely lively event in which more than 4000 attendees from all over the world came together once again. They saw the first-hand results of several major trials and large clinical studies that changed clinical practice.</p>
<p>This year’s ESO Summer School Program was hosted by the Caen University Hospital Department of Neurology. The Caen Neurology team organized three-day seminar featuring lectures on acute stroke care, small vessel disease, secondary stroke prevention, and organizational structures in stroke care. Also several translational lectures on new and exciting areas of stroke research were featured throughout the summer school program.</p>
<p>Later in the year, during the 6th Garmisch ESO Stroke Science Workshop, leading stroke scientist gathered to share their newest insights in their field of research. This edition of the Workshop proved that we can have lively, in-dept discussions from our home offices. In addition, the lectures have been made available to all ESO members in the ESO educational platform – eSTEP.</p>
<p>Looking forward to 2022, we will be launching the PhD Series on the ESO Blog. For this series we will interview PhD student in different stages of their PhD program and ask them about the challenges they encounter and how to make your PhD program a success.</p>
<p>The abstract submission for ESOC 2022 is already open. Although this will be a hybrid meeting, the opportunity to meet in person in Lyon is much awaited. If you are an early career researcher designing a new study, don’t miss the opportunity to discuss your protocol or preliminary results in the Young Investigator Research Design Workshop. Those selected for oral communication will receive constructive assessment and advice by a senior, internationally-renowned invited investigator. Also, two prizes will be awarded to the best presenters.</p>
<p>We would like to express our appreciation to all the YSPR committee members and the ESO community for participating so enthusiastically in all these activities, on location and virtually. Although this new pandemic era has given rise to new challenges, it has also demonstrated that by working together, we will continue to successfully battle stroke worldwide.</p>

<p><strong>Session Report</strong></p>
<p>The session at the ESO 2021 virtual conference, <a href="https://cslide.ctimeetingtech.com/esoc21/attendee/confcal/show/session/43" target="_blank" rel="noopener">Acute Stroke Treatment in Difficult Clinical Situations: In-Hospital Strokes and Other Challenges</a> was one of several sessions that addressed stroke in COVID-19 patients. Francesco Corea, Chair of ESO`s Social Media Committee and an Acute Stroke Specialist, summarises the session below.</p>

<p>Thrombolysis in Patients With Recent Surgery or Trauma</p>
<p>Gordian Hubert presented the talk on Thrombolysis in Patients with Recent Surgery or Trauma. A relevant topic, as he points out, but with little evidence to support our decisions. The experiences of the telestroke network TEMPiS may be encouraging, showing a low rate of surgical site hemorrhages. Recent surgery and major surgery and especially the combination of both seem to have the highest risk of surgical site hemorrhage. But given the retrospective design and general low evidence, decisions have to be made case by case. In post-traumatic patients evidence is even more scarce. He proposed an approach on how to find decisions in these cases: a) define the risk of surgical site hemorrhage, b) know the damage a bleeding could cause and c) know about possible interventions.  Further research is encouraged in this area.</p>
<p><strong>Marjaana Tiainen</strong></p>
<p>The Older Patient With Extensive Medical History: What to Do?</p>
<p>The speaker Senior neurologist working in Stroke Unit and Emergengy Medical services at Helsinki University Hospital addressed the topic of stroke in older and complex patients. In these cases comorbiity index may be useful. Thus we can define the concept of frailty with specific related scales. Together with scoring scales to select the patients also an appropriate evaluation of the premorbid functional performance of the patient may be vital for the decision making process. The pre-notification and accessibility of electronic medical charts may simplify the management of stroke admissions. Functional performance may be relevant for the decision making process to speed up treatment. Not living alone and having impaired mobility with limited functionality may need to better select candidates to IVT EVT.</p>
<p><strong>Rajiv Advani</strong></p>
<p>Stroke in the Cath Lab and Related Situations</p>
<p>The third speaker Rajiv Advani depicted how the diffusion of endovascular procedures in many hospitals is increasing all over the world. The complications of cardiac procedures is low but in absolute numbers can be relevant in daily practice. Both ischemic and hemorrhagic event may complicate the hospitalization and need for an urgent neurological evaluation. These patients are often in advanced age thereby are selected for endovascular procedures instead of undergo to traditional open heart surgery. The detection of stroke during the procedure can be challenging for various reasons such as the setting of the patients in a ICU and multi-morbidity. An appropriate balance of risks and benefits is need. Main risks are due to GE tract bleedings. The use of iv salicylates or abciximab seem not safe for higher risk of bleedings after IVT. The recent use of heparins needs monitoring of aPTT. TWhile in general thrombolysis using reduced doses or rTPA have no sufficient data available to state a reduced bleeding risks. Direct EVT may be indicated in selected cases with large vessels occlusion.</p>
<p><strong>Marco Bacigaluppi</strong></p>
<p>Thrombus Composition in Diverse Stroke Aetiologies: Imaging, Histology and Treatment.</p>
<p>Dr Bacigaluppi continued the session with a presentation from Stroke Unit and Neuroimmunology lab of San Raffaele Hospital in Milan. His data demonstrated relevant informations from the analysis of thrombi retrieved after EVT. Thrombus analysis, can be made by many pathologists, and may offer relevant data but only in a small amount of subjects (3% of all stroke cases, because limited to those with large vessels pathology undergoing EVT). The analysis of the sample may offer interesting informations on: etiology, imaging correlations, new therapeutic targets/devices and blood biomarkers.</p>
<p>The point is to unravel stroke etiology looking in the sample for a “signature” on the origin: colesterol, structure, calcification, prostethic tissues, bacteria.</p>
<p><strong>Claudio Baracchini</strong></p>
<p>Management of Acute Ischemic Stroke in Patients Suspected of COVID-19 Infection</p>
<p>The last speaker from Padua University Hospital Claudio Baracchini reported interesting data on the incidence of stroke in COVID pandemic series, which was initially reported up to 5 % but later in lower but not negligible figures.</p>
<p>Potential mechanisms underlying are various: coagulopathy, inflammation, cardiopathy, platelet abnormalities. Stroke patients are more prone to develop COVID in a severe way.</p>
<p>As well as many COVID 19 related strokes seems crypto-genetic and occurring in abnormal age with higher mortality rates and poorer outcome. During pandemic times a protected stroke code was recommended to keep a fast safe lane together with  temporary stroke buffer areas and mobile CT unit. No general contraindication to stroke specific treatments in acute a secondary prevention in COVID stroke cases. The use of tenecteplase may reduce staff exposure.</p>

In the last two years, we have found a new way of keeping close to each other, shortening distances, facing everyday problems with resilience and creativity, because adaptation is in our nature.</p>

Here you can find interesting results about brain network rearrangement after stroke and about physiological mechanisms that are possible future targets for stroke rehabilitation (read “<a href="https://cslide.ctimeetingtech.com/esoc21/attendee/eposter/poster/1630?r=snm%7E135" target="_blank" rel="noopener">Somatotopy of the interhemispheric interactions reflected in physiological mirror activity: preliminary results” by Nazarova et al</a>.</p>
<p>Talking about rehabilitation, you might want to explore the <strong>AS04 CLINICAL TRIAL RESULTS – REHABILITATION &amp; RECOVERY</strong> section. If you already know that dysphagia is the main culprit of stroke-related pneumonia, you might want to read about possible technology-based assessment and treatment of swallowing impairment, such as “<a href="https://cslide.ctimeetingtech.com/esoc21/attendee/eposter/poster/1372?r=snm%7E121" target="_blank" rel="noopener">Swallow safety, timings and clearance: do multiple measures detect treatment effects? Analysis from the swallowing treatment of electrical pharyngeal stimulation (STEPS) trial</a>” by Benfield et al and “<a href="https://cslide.ctimeetingtech.com/esoc21/attendee/eposter/poster/417?r=snm%7E121" target="_blank" rel="noopener">Randomised controlled feasibility trial of swallow strength and skill training with surface electromyographic biofeedback in acute stroke patients with dysphagia</a>” by Everton et al. In the same section, interesting findings of the pilot trial “<a href="https://cslide.ctimeetingtech.com/esoc21/attendee/eposter/poster/769?r=snm%7E121" target="_blank" rel="noopener">Technology-supported sitting balance therapy versus usual care in the chronic stage after stroke: a pilot randomized controlled trial</a>” by Thijs et al provide new insights into sitting balance therapy.</p>
<p>Although the future seems quite pale in these hard times, remember that new technologies, informatics and artificial intelligence are getting stronger and stronger and can provide new diagnostic and prognostic tools. The virtual format of the conference truly allows to view an extensive selection of E-posters from the comfort of one’s home.</p>
</div></section></p></div><section  class='av_textblock_section av-khbu4f9j-228ce13fdf71c131fbc82c2dc5200010 '   itemscope="itemscope" itemtype="https://schema.org/BlogPosting" itemprop="blogPost" ><div class='avia_textblock'  itemprop="text" ><p>Several E-posters caught my eye in the rapidly expanding field of heart-brain research. Prolonged cardiac monitoring for the detection of atrial fibrillation is an essential part of etiological workup in ESUS patients. However, this may not be possible in all patients in all settings. The authors of <strong>FREQUENT PAROXYSMAL ATRIAL COMPLEXES AND SUPRAVENTRICULAR TACHYCARDIA COULD GUIDE THE DIAGNOSIS OF HIDDEN ATRIAL FIBRILLATION IN PATIENTS WITH ESUS </strong>(Sanchez Fernandez <em>et al.)</em> have shown that cardiac rhythm biomarkers such as frequent atrial paroxysmal complexes and paroxysmal supraventricular tachycardia may indicate a higher rate of occult atrial fibrillation in stroke patients and therefore may be useful in selecting patients for prolonged cardiac monitoring.</p>
<p>Cardiac CT has become a useful tool in the etiological workup of stroke.  In <strong>SEQUENTIAL CARDIAC CT TO DETERMINE CARDIAC THROMBI DISSOLUTION RATE IN ACUTE ISCHEMIC STROKE </strong>Beemsterboer <em>et al. </em>have evaluated the cardiac thrombi dissolution rate during the acute phase of stroke. They demonstrated that cardiac thrombi may resolve within days of stroke occurrence, thus cardiac thrombi might not be detected on later imaging.  Early cardiac imaging therefore may improve detection of cardioembolic source in stroke.</p>
<p>Treating patients with lacunar stroke can be challenging as they may show early neurological deterioration (END). The authors of <strong>EARLY NEUROLOGICAL DETERIORATION IN LACUNAR STROKE: CLINICAL AND IMAGING PREDICTORS AND ASSOCIATION WITH LONG-TERM OUTCOME </strong>(Vynckier <em>et al.</em>) showed that END is common, occurring in 1 in 6 lacunar stroke patients. They demonstrate radiological and clinical variables that are associated with END including lower NIHSS on admission, capsular warning syndrome, ventral pontine infarction and hypoperfusion on imaging. Importantly, acute dual antiplatelet therapy was associated with reduced risk of END in this study.</p>
<p>Recanalization therapy of stroke in the prolonged time window currently requires penumbral imaging. However, this is not feasible in all settings.  Several studies evaluated surrogate markers on native CT imaging for selecting patients eligible for treatment in the extended time window.  In <strong>CEREBRAL ATTENUATION CHANGES ON NON-ENHANCED CT: A SURROGATE FOR CT PERFUSION–BASED PENUMBRAL TISSUE AND INFARCT CORE IN ACUTE ISCHEMIC STROKE </strong>(Alzahrani <em>et al.</em>) the authors showed that specific brain attenuation ratios on native CT can differentiate penumbral and core lesions. Based on the findings in <strong>CURRENT ROLE OF ADVANCED NEUROIMAGING AND ITS CORRELATION WITH NON-CONTRAST COMPUTED TOMOGRAPHY IN THE LATE WINDOW TREATMENT SELECTION</strong> (Rodrigo-Gisbert <em>et al.) </em>the authors suggest that simplified imaging in stroke patients in the extended time window may be reasonable in patients with an ASPECTS score ≥6 on non-contrast CT imaging.</p>

Although there was no live chatting with the presenters, they still were able to clearly present the results of their hard work.</p>
</div></section></p></div><section  class='av_textblock_section av-khbu4f9j-228ce13fdf71c131fbc82c2dc5200010 '   itemscope="itemscope" itemtype="https://schema.org/BlogPosting" itemprop="blogPost" ><div class='avia_textblock'  itemprop="text" ><p>Many studies focused on pre-hospital stroke care. An interesting study that caught my eye was presented by Ramos Pachón et al. from Spain. <strong>MEDICAL COMPLICATIONS DURING TRANSFER IN DRIP-AND-SHIP VS MOTHER-SHIP CIRCUITS: SECONDARY ANALYSIS OF RACECAT TRIAL</strong>. These authors found that patients in the mothership protocol had an increased risk of transfer complications (e.g. vomiting, GCS declining, RACE score worsening) compared with the drip-and-ship protocol. Transfer complications resulted in a higher odds of mortality and a worse clinical outcome.</p>
<p>Earlier identification of large vessel occlusion (LVO) patients was addresses is several studies. <strong>TELESTROKE: REAL-TIME VIDEO ANALYSIS ALLOWS IDENTIFICATION OF LVO IN PATIENTS WITH SUSPECTED STROKE</strong> was presented by S. Thielmann from Switzerland. This feasibility study in 97 suspected stroke patients concluded that their telestroke concept is feasible and that early identification of LVO-patients could be possible. This could lead to reduced onset-to-treatment-time, preparing for endovascular treatment, and an improved clinical outcome. I am very excited to learn the results of the planned main study that will start in 2022.</p>
<p>Another study that focused on early detection of LVO patients was the study by van Meenen et al from the Netherlands, <strong>DETECTION OF LARGE VESSEL OCCLUSION STROKE WITH ELECTROENCEPHALOGRAPHY IN THE EMERGENCY ROOM: FIRST RESULTS OF THE ELECTRA-STROKE STUDY</strong>. The authors present the results of the first 100 patients who presented to the emergency department with suspected or confirmed stroke and underwent dry electrode EEG. Of the 65 patients with sufficient data quality to perform the analysis, 35 had acute ischemic stroke and 9 had LVO of the anterior circulation. The EEG feature with the highest diagnostic accuracy for LVO stroke was theta-alpha ratio. The authors concluded that dry electrode EEG seems a promising tool for detection of LVO, although the quality of the data needs to be further improved and evaluation in the prehospital setting is also needed.</p>
<p>Another interesting study that caught my attention was presented by Krebs et al from Austria. <strong>CT- VERSUS MRI-BASED IMAGING FOR INTRAVENOUS THROMBOLYSIS IN ISCHEMIC STROKE</strong>. This observational study focused on the choice of initial imaging modality. The authors found that this did not have an effect on safety nor functional outcome after stroke treatment, although the door to needle time was considerably longer in the MRI group.</p>

His research activities have focused on stroke epidemiology, stroke syndromes, small vessel disease, ultrasound, neuroimaging, clinical genetics, clinical trials and organisation of stroke services. He is author of more than 500 scientific publications with an H-index Web of Science All Databases of 98, about 80 000 citations and a Google scholar index of 103. He is not only a founding member of the ESO and was its Vice President from 2008 to 2010, but also serves the society in his function of Editor-in-Chief for the European Stroke Journal ESJ. From 2008 to 2012, he acted as President of the WSO. In addition, Bo created the world’s first national stroke register The Swedish Stroke Register (Riksstroke) in 1995 of which he is currently Chair. As member of the Steering Committee, he is highly engaged in the ESO flagship project ‘Stroke Action Plan for Europe SAP-E’, and was one of the first authors of the scientific publication in 2018. Bo Norrving is a Fellow of the ESO, the WSO, the AHA, and the RCPE.</p>

As a clinical researcher, his main research interest involves diagnosis, management, treatment and outcome of patients with acute neurological diseases, especially of patients with acute ischaemic and haemorrhagic stroke. He is participating in multiple preventive, diagnostic and acute treatment trials and stroke registries and acts as principal investigator of several international multicentre academic trials on the management of patients with ischaemic and haemorrhagic stroke. Urs is the former Secretary General of the ESO, Treasurer of the Swiss Neurological Society (SNG), Member of the Programme Committee of the European Academy of Neurology (EAN), and he is Co-Founder the ESO ESMINT ESNR Stroke Winter School. Urs Fischer is a Fellow of the ESO.</p>

From 2012 to 2014, he was President of the ESO and from 2018 to 2020, he was President of the WSO. Michael founded the first acute stroke unit in Austria in 1997 and co-founded the National Stroke Unit Network.  From 2007-2021 he co-chaired the European Master’s Program in Stroke Medicine at the Danube University in Krems, a two-year program of excellence. In addition, he reported to the WHO and WHA, represented the WSO at the meetings and for activities of the Global Coalition for Circulatory Health and the NCD Alliance. <p>A mentor should provide clear guidance to the mentee, not only from a research or career perspective but also from a personal one. Conversely, a mentee should provide clear goals, and initial steps on how to reach those goals. This way, it is easier for both the mentor and mentee to work on achieving those goals.</p>

<p>The session took place on <strong>Wednesday 1<sup>st</sup> September at 08.30 in Hall A </strong>and will be avaliable to watch on demand via the ESOC platform until 3 December 2021.</p>
<p>In this series of interviews, we are going to meet them and hear about their stories.</p>

<p>I obtained my degree in Medicine at the University of Genoa in Italy in 2016. During my university studies I also completed the prestigious Biomedical Excellence Programme at the Institute of Superior Studies of the University of Genoa (ISSUGE). Also, with the added experience received after a research stay at the Medizinische Hochschule Hannover (MHH), which focus their research on peripheral nerve regeneration under the guidance of Prof. Kirsten Haastert-Talini, I achieved the certification for my research training programme in Neurology at the University of Genoa in 2016 under the guidance of Prof. Angelo Schenone.</p>
<p>I commenced my specialist training in neurorehabilitation in Pavia, Italy and then moved to Switzerland where I am currently working as a clinical researcher at the CHUV University Hospital and University of Lausanne where I am completing my specialist training as a neurologist together with an MD-PhD degree in stroke neurology at the University of Lausanne (UNIL) under the guidance of Prof. Patrik Michel.</p>
<p>My aspiration is to become an academic clinical researcher, developing expertise in the fields of stroke neurology and of neuroimaging in stroke. Hence, my main interests are in these two fields, focusing especially on acute stroke management, perfusion neuroimaging and outcome prediction.</p>

<p>I first started to get involved in stroke research during my neurorehabilitaton training focusing on an ongoing project which has the aim to establish the role of blood biomarkers in stroke. After moving to Switzerland, I have had the chance to further develop my interest in stroke research by working as a clinical researcher taking care of the ongoing clinical trials at the Lausanne Stroke Center and, in parallel, to pursue a project on the development of prognostic assessment tools in acute ischemic stroke patients.</p>

<p>Stroke has a high burden of disease representing one of the main causes of mortality and long-term disability worldwide. It is extremely time-dependant and sequelae can be greatly minimized if rapid and efficient clinical management are put in place. Hence, research in such field has the possibility to significantly make an impact on outcome and improve patient care.</p>

<p>The first step has been to enter a competing and reputed specialist training programme in one of the top leading research centers worldwide. Secondly, research costs and requires funding. Thereby, one of the most challenging tasks is to shape and develop a strong project and to be able to put it in evidence to be able to get the necessary funding.</p>

There should be a mutual trust enabling the development of progressive competencies and responsibilities. Your commitment and dedication together with a protected time for research will be boosted thanks to his network and collaborations, infrastructure and initial financial support for research projects. Helping you to develop your capabilities and competencies will help you develop your career as an established expert in your field.</p>
<p>You as a mentee should therefore expect that he can share his experience in order for you to best face your own situations. You should give trust and show commitment and dedication to the work, expecting to be progressively more and more accountable.</p>

<p>Intracerebral hemorrhage (ICH) is an area of stroke medicine where there are still a number of unanswered questions when it comes to pathophysiology and prognosis. I developed an interest in ICH during my years in residency and wanted to continue my research in this area during my fellowship training.</p>

<p>I’ve been very fortunate in regards to how my career has developed thus far. With that being said, an ongoing personal challenge is being comfortable with my own level of productivity and not comparing myself to others. Acknowledging the fact that everyone’s academic journeys develop at different paces is an important lesson that I try to remind myself of.</p>

<p>At present, I try to ensure that my time at home in the evenings is dedicated to my family. If in the event that I have a busy week and work in the evening is required, I try to plan this in advance with my family. If I am being honest however, I am still learning how to strike the balance between work life and home life. My family has been very generous and have supported me when I need to work during off-hours.</p>

<p>In my experience, successful mentors are able:</p>
<p>*   To guide mentees to develop projects that the mentee is passionate about, but also help develop projects which are feasible to perform</p>
<p>*   Look out for unique opportunities for their mentees</p>
<p>*   Be available for mentees to provide honest advice on career options and opportunities</p>
